The Essential Plan is perfect for individuals or families seeking consistent annual health assessments and reliable timely access for all medical concerns, such as annual health physicals and standard sick visits. This plan most resembles the frequency and type of visits often seen by standard primary care practices (but with a more personal touch.)
The Transform Membership
An all-inclusive package intended to jump start a complete health overhaul. Includes detailed fitness and nutrition assessments with licensed experts, as well as home tools to track health metrics and the follow-up support needed to ensure you achieve your health goals. Ideal for individuals looking to build a comprehensive health profile and achieve transformative health improvements over the course of twelve months.

There is a one-time Initial Visit Fee
of $150 on monthly memberships.
Osteopathic Manipulative Treatment (OMT)
Pricing:
Membership includes two OMT visits per year. Additional treatments are $125 per session.
Non-member consultation and first treatment $250. Additional treatments are $175 per session.
OMT sessions are generally 30-40 minutes, while the first visit may be up to an hour. They are all unique and tailored to each patient’s needs.
